中老年人股骨转子间骨折动力髋螺钉内固定术疗效分析

Clinical Evaluation of Femoral Intertrochanteric Fractures with Internal Fixation of DHS Among Middle and Old Aged Patients

目的探讨中老年人股骨转子间骨折经动力髋螺钉内固定的方法及临床疗效.方法在C型臂X线机透视下闭合复位,应用动力髋螺钉(DHS)内固定治疗中老年人股骨转子间骨折31 例,其中男23 例,女8 例;年龄49～65 岁,平均57.3 岁.结果术后随访9～17个月,平均13.1个月.骨折均达到满意复位并愈合,无髋内翻,髋螺钉切割、拔出或穿透股骨头现象.结论中老年人多存在不同程度的骨质疏松,应用DHS内固定治疗股骨转子间骨折,固定牢,创伤小,术后并发症少,疗效满意.

Objective To explore the techique and clinical effects of internal fixation with dynamic hip screw （DHS） for femoral intertrochanteric fractures among middle and old aged patients. Methods Under X-ray guidance, 31 cases of femoral intertrochanteric fractures were performed closed reduction and percutaneous dynamic hip screw （DHS） internal fixation（23 males and 8 females, range 49-65 years with an average of 57.3 years）. Results All the patients achieved solid fusion during the followed up time 9-17 months with an average time of 13.1 months. All the femoral intertrochanteric fractures were reduced satisfyingly,there was no coxa varus, screw incising, pulling out or penetrating into articular space. Conclusion Among middle and old aged patients with different degree of osteoporosis, internal fixation with dynamic hip screw （DHS） for femoral intertrochanteric fractures has advantages of minimal invasion, stable fixation, few complications and satisfying clinical effects.
