老年股骨粗隆间骨折三种固定方法疗效分析

Senile Intertrochanter Fracture of Femur: A Therapeutic Effect Analysis on Three Fixation Methods

目的研究外固定支架、股骨粗隆解剖钢板、DHS治疗股骨粗隆间骨折的临床疗效。方法对44例老年股骨粗隆间骨折,分别采用外固定支架、股骨粗隆解剖钢板、DHS固定治疗,对术中及术后愈后情况进行观察分析。结果本组病例术后随访1-2年,一般8-12周骨折临床愈合。结论稳定型骨折(Ⅰ、Ⅱ型)宜采用外固定支架和DHS治疗,外固定型骨折(Ⅲ、Ⅳ型)宜采用股骨粗隆解剖钢板治疗,能提供即刻稳定,有利于骨折愈合,但有创伤大、失血多、手术时间长等缺点。

Objective To study the clinical therapeutic effects of external fixation of stand, intertrochanter anatomic steel plate and DHS. Methods 44 cases of senile intertrochanter fracture of femur were respectively treated with external fixation with stand, intertrochanter anatomic steel plate and DHS. We observed analyzed the situations during and after the operation and after the healing. Result The one-to two- year followup survey found the group of patients got healed in 8 to 12 weeks. Conclusion It is advisable to adopt the external fixation with stand and DHS for stable fracture（type Ⅰ, type Ⅱ）;it is advisable to adopt the in tertrochanter anatomic steel plate for external fixation fracture（type Ⅲ, type Ⅳ） which ensures timely stability and convenient fracture healing, but gives rise to large wound area, profuse bleeding and long duration of operation.
