血清α－L－岩藻糖苷酶与甲胎蛋白联合检测在原发性肝癌诊断中的临床价值探讨

Combined Assay of Serum AFU and AFP in Patients with Primary Hepatic Carcinoma

采用改进的Ｔｒｏｏｓｔ法对９７例正常人、１１６例原发性肝癌（ＰＨＣ）、２１例肝硬化、２１例转移性肝癌、３２例其它消化道恶性肿瘤、１２例肝良性病变患者血清α－Ｌ－岩藻糖苷酶（ＡＦＵ）活性进行了测定。结果ＰＨＣ组血清ＡＦＵ活性明显高于其它各疾病组，更高于正常人组（Ｐ＜０．０１）。对ＰＨＣ诊断敏感性为７５％，特异性为９０．１％，在３６例甲胎蛋白（ＡＦＰ）阴性的ＰＨＣ中，ＡＦＵ阳性率为８０．６％。ＡＦＵ与ＡＦＰ联合检测，可使ＰＨＣ的诊断阳性率提高到９３．９％。

The activity of serum alpha-L-fucosidase(AFU) in 116 patients with primary hepatic carcinoma(PHC), 21 hepatic cirrhosis,21 metastatic hepatic cancers,32 malignant cancers of digestive tract and 12 benign bepatic diseases and 97 normal subjects was determined with modified Troost method.The results showed that serum AFU activity in PHC patients was higher than that with other disease and markedly higher than in normal controls(P<0.01).In PHC patients,the sensitivity rate was 75%,specificity 90.1%.In 36 PHC patients in whom AFP was negative,the positive rate for AFU was 80.6%.The accuracy rate in PHCcombining positivity of AFU and AFP was 93.9%.
