腹部闭合性创伤合并颅脑损伤87例诊治分析

The diagnosis and treatment of 87 cases with abdominal closed injury with craniocerebral trauma

目的总结腹部创伤合并多发伤诊治经验.方法选择1994～2003年闭合性腹部创伤合并颅脑损伤的多发伤87例分析.男54例,女33例;共损伤脏器101个,全部病例均行手术探查.结果本组治愈59例,死亡28例.结论对于有低血压及意识丧失的患者,为减少腹部的漏诊,应反复、多点腹腔穿刺,尽可能利用B超和CT在腹内脏器伤诊断中的重要作用.在治疗上应先治疗危及患者生命的损伤,先积极抗休克治疗,并且要重视颅内高压的治疗.

Objective To summarize the experience of diagnosis and treatment of abdominal closed injury with multiple trauma. Methods We chose and analyzed all the 87 cases multiple trauma woundeds of abdominal closed injury with craniocerebral trauma from 1994 to 2003.54 cases were male, 33 cases were female. 101 visceras were traumatized,all the cases were operated. Results The 59 cases woundeds restored health, the 28 cases wounded died. Conclusions We should puncture repeated and many points,and use “B” and “CT” as possible to diagnose the trauma of abdominal viscera,especially to the low blood pressure and lost consciousness wounded. In treatment, we should treat the trauma risk on the life earlier,try our best to contend with the shock,and attach importance to the treatment of the increased intracranial pressure.
