曲美他嗪治疗慢性充血性心力衰竭的临床观察

The efficacy of trimetazidine in chronic congestive heart failure

目的观察曲美他嗪(trimetazidine,TMZ)对慢性充血性心力衰竭患者的临床药效及不良反应.方法 115例慢性充血性心力衰竭(NYHA心功能分级Ⅱ～Ⅳ级)患者随机分为两组.对照组(n＝50例)应用常规药物,曲美他嗪组(n＝65)在常规药物治疗的基础上加用曲美他嗪,用药期12周,定期门诊随访观察治疗前后心功能分级及超声心动图检查等.结果曲美他嗪组总有效率(92.3%)明显优于对照组(66.0%)(P<0.01);曲美他嗪能明显改善慢性充血性心力衰竭患者临床症状和体征,左室射血分数(LVEF)和左室短轴缩短率(FS)明显提高(P<0.01或0.05),且无严重不良反应出现.结论慢性充血性心力衰竭患者在常规治疗基础上,加用曲美他嗪可进一步改善心功能,对治疗慢性充血性心力衰竭是一种安全有效的方法.

Objective To observe the clinical effects and side effects of trimetazidine in chronic congestive heart failure patients. Methods 115 congestive heart failure patients were randomly divided into two groups. The control group（ n = 50） was given the routine medicine. The trimetazidine group（ n = 65） was administered trimetazidine on the basis of routine treatment. The therapy period was 12 weeks. We observed the heart functional classification and echocardiography regularly. Results The total effect rate of trimetazidine group was 92.3%. It had better outcome than the control group（66.0 % ,P 〈 0.01 ）. Trimetazidine could improve the signs and symptoms of chronic congestive heart failure. The left ventricular ejection fraction and short axis shortening rate were improved significantly（P 〈 0.01 or 0.05）. There was no serious side effect in the trimetazidine group. Conclusion Trimetazidine may improve the heart function of chronic congestive heart failure patients on the basis of routine treatment, which is a safety and effectual method to treat chronic congestive heart failure.
