摘要 目的:探讨肾上腺弥漫性增生的CT诊断标准。材料和方法:回顾性分析临床及实验室检查诊断的50例弥漫性肾上腺增生患者肾上腺的CT表现,并测量其体部及肢体的最大厚度,将其与26例正常人的肾上腺测量值进行比较。结果:肾上腺弥漫性增生组与正常对照组在双侧肾上腺体部及肢体最大厚度方面存在显著统计学差异(P<0.05);以“肢体厚度大于5mm”作为诊断标准不具有明显优越性;以左侧肾上腺体部最大厚度作为诊断标准所得的ROC曲线下面积最大(0.923)。结论:“左侧肾上腺体部最大厚度大于0.795cm”可能可作为肾上腺弥漫性增生的定量诊断标准。 Purpose: To discuss the diagnostic criterion of diffuse adrenal hyperplasia in multi - detector CT.Materials and Methods: The CT features of 50 cases of diffuse adrenal hyperplasia , which were confirmed by clinical and lab examinations, were retrospectively analyzed. The mean body and limb width of diffuse adrenal hyperplasia were recorded and compared with the one of 26 healthy control subjects . Results: The adrenal glands in diffuse adrenal hyperplasia were significantly larger than those in healthy control subjects (P 〈 0.05). A specificity of 100% was not achieved when the mean limb width was 5 mm or greater. Receiver operating characteristic curve analysis showed the left adrenal body width in detecting diffuse adrenal hyperplasia was the best. Conclusion: The mean left adrenal body width of greater than 0. 795 cm may be used to diagnose diffuse adrenal hyperplasia.
