Analysis of maternal death of 21 patients

目的回顾分析我院20年间孕产妇死亡原因,提出进一步降低孕产妇死亡率的措施.方法将我院1983年1月～2002年12月共20年21例孕产妇死亡资料以产科直接原因和产科间接原因按先后两个 10年作为两个阶段对比.结果经加强孕期保健和孕期并发症、合并症的防治,死亡病种和死亡人群发生了变化,第二阶段产褥感染、妊娠合并先心病无死亡,但又出现了异位妊娠、肺栓塞等新病种.本地人口死亡减少,外地人口死亡上升.结论拓宽防治工作范围,加强外地人口管理,加强健康教育,提高孕产妇保健的系统性和服务质量,加强临床医生专业知识和急救技能培训是降低孕产妇死亡的关键.

Objective To assess the causes of maternal death occurring over an eighteen- year period （ 1983 to 2002） in our hospital and explore the preventive measures to reduce the maternal death. Methods Twenty - one maternal women died from January 1983 to December 2002, they were divided into two stages according to two nine - year periods： stage I （January 1983 to December 1992）, stage H （January 1993 to December 2002）. The direct and indirect causes of maternal death were compared between the two stages. Results By providing better maternal health care, preventing and treating complications and associated diseases than in stage I , no death occurred in patients with puerperal infection and congenital heart disease in stage H , but some diseases were observed, such as ectopic pregnancy, pulmonary embulism etc. The maternal death was higher in patients who had not undergone maternal health care than patients receive health care. Condusions Broadening the range of prevention and treatment and strengthening the administration of people out of town is an important way to reduce maternal death. Strengthening health education, improving maternal health care systemically, providing adequate obstetrical emergency service and improving medical staff＇ s skill are effective to decrease the maternal death.
