脊柱植骨融合核素显像的实验研究 被引量:2

The basic research of bone graft fusion in spinal surgery using radionuclide bone imaging

摘要 目的利用核素骨影像建立新的评价脊柱植骨融合的标准,从而较为客观地评价脊柱部位的植骨融合情况,为脊柱植骨融合患者术后正确科学地功能锻炼提供理论依据。方法将30只中国大白兔行脊柱植骨融合术,随机分为4组,分别于术后2周,4周,6周8、周行X线片及放射性核素骨显像检查,并与病理学切片对比植骨融合情况,研究其精确性。结果(1)实验组相应植骨区域放射性计数与相邻椎体之比为1.80±0.05(-x±s,n=6);术后各周,实验组植骨区放射性比值均大于1.05,2周植骨区开始出现放射性浓聚,4周达到高峰,6周下降,但仍高于对照组。(2)与病理结果对应分析,同位素骨扫描评价植骨融合率较X线敏感。结论同位素骨扫描可以很好评价脊柱植骨融合的成活情况,并且较X线敏感。 Objective To establish a new criterion to judge of bone fusioning by using. It is very imprtant to evaluate the degree of bone fusion in spine, afford theory basis of right and scientific function exercise for those patients who suffered hone fusion in spine. Methods The china rabbit were operated using same spinal bone graft fusion after they were. naesthesia.Then, they were divided four group in random , and examined using X-ray and bone imaging in 2 weeks, 4 weeks, 6 weeks and 8 weeks after surgery. Besides, there was a black comparsion group. The animal was execute when the examination was finished. Afterwards, the pathology examination were done after the sample were exsected in bone graft zone. At last, all resuh were collected including pathology slice, X-ray film, and radionuclide bone imaging, and analysed using blind method. Results (1) In experiment, the ratio of radionuclide count is 1.80±0.05 between the hone graft zone and the adjacent centnum; the ratio of radionuclide count is all higer than 1.05 in every week;The radio activty of the bone grafts increased in the second week, and reached the highest in the fourth week, then dropped gradually back to normal at the sixth week, but higher than the comparison group. (2)The result of monitoring for the bone transplantation by radionuclide bone imaging is accorded with histomorphometry, comparing with X-ray, it is more accurate and direct. Conclusion 1. Radionuclide bone imaging was the chief method for early assessment of the survival of mandibular bone grafts. 2. The result of monitoring for the bone transplantation by radionuclide bone imaging is accorded with histcmorphometry, comparing with X-ray, it is more accurate and direct.
