大剂量甲氨蝶呤治疗儿童急性淋巴细胞白血病的血药浓度测定方法及结果分析 被引量:2

摘要 目的采用高效液相色谱法(HPLC)测定患儿甲氨蝶呤(MTX)的血药浓度,探讨大剂量甲氨蝶呤(HD-MTX)用药后54小时内血药浓度的变化,以指导临床合理用药。方法应用大剂量甲氨蝶呤加四氢叶酸钙(HD-MTX/CF)解救治疗6例小儿急性淋巴细胞白血病(ALL),治疗分4个疗程,每疗程按MTX 5g/m2给药,间隔2周,同时进行用药前和用药后24、36、424、8、54小时血药浓度监测。结果6例患儿24小时血药浓度均在1×10-4mol/L-1×10-5mol/L之间,36-54小时的血药浓度在1×10-6mol/L×10-7mol/L之间。结论在血药浓度监测下,使用大剂量MTX加CF解救治疗是安全有效的。 Objective To establish an HPLC method for the determination of plasma Methotrexate(MTX) concentrations in children with acute lymphoblastic leukemia(ALL) treated with high-dose MTX(HD-MIX) and Calcium Folinate (CF) revue and to investigate plasma MTX concentrations in 54h after HD-MTX administration. Methods HD- MTX/CF rescue therapy was administered to 6 children with ALL.The therapy consisted of 4 cycles of HD-MTX(5g/m^2)]CF rescue with 2- week internal between cycles. The established HPLC was used to monitor the plasma MTX concentrations before and 24, 36, 42, 48, 54h after the administration in each cycle. Results The plasma MTX concentrations in the children ranged from 1×10^-4 to 1×10^-5 mol/L at 24h and were between 1×10^-6 to 1×10^-7 mol/L from 36 to 54h after the administration. Conclusion The HD- MTX/CF rescue therapy administered to the children with ALL. is safe and effective, with the monitoring of plasma MTX concentrations by HPLC.
