载脂蛋白E基因型多态性与青年高血压性脑出血 被引量:2

摘要 目的:观察青年高血压性脑出血患者的载脂蛋白E基因型的分布,探讨其分布与高血压性脑出血的关系。方法:高血压性脑出血组120例选自河源市人民医院及南方医院2001-03/2004-03的住院患者,入院距发病时间1h~3d并经CT检查明确诊断。所有患者均有高血压病史,且在3个月以上,年龄18~45岁。选择120名自愿健康体检者为对照组。采用聚合酶链反应酶切和聚丙烯酰胺凝胶电泳技术检测分析两组载脂蛋白E基因分型和等位基因分布。结果:按意向处理分析,240例均进入结果分析。①高血压性脑出血患者载脂蛋白E基因型E2/4,E3/4,E4/4明显高于对照组(P<0.01),E3/3明显低于对照组(P<0.01)。②高血压性脑出血患者等位基因频率以E3最多,E4次之,E2最少(40.83%,37.92%,21.25%)。对照组等位基因频率以E3最多,E2次之,E4(76.67%,15.00%,8.33%)最少。③与对照组相比,高血压性脑出血患者等位基因E4的频率明显升高,E3频率明显降低(P<0.01)。结论:高血压性脑出血患者E4的频率明显高于对照组,E3的频率明显低于对照组,提示E4为青年高血压脑出血的危险因素,E3可能具有一定保护作用。 AIM: To observe the distribution of genotype of apolipoprotein E (ApoE) in youth patients with hypertensive cerebral hemorrhage, to explore the relationship between its distribution and hypertensive cerebral hemorrhage. METHODS: 120 inpatients in hypertensive cerebral hemorrhage group were selected from Heyuan People's Hospital and Nanfang Hospital from March 2001 to March 2004. The time of hospitalization was 1 hour to 3 days apart from the time of onset and CT examination was used to diagnose surely. All the patients had the history of hypertensive disease, and over 3 months aged from 18-45 years old. 120 healthy voluntary testees were selected as control group. The polymerase chain reaction (PCR) and polyacrylamidedel electrophoresis technique were used to detect and analyze genotyping and allelomorph distribution of apolipoprotein E in the two groups. RESULTS: According to intention-to-treat analysis, 240 patients were all involved in the result analysis.① The genotype E2/4, E3/4, E4/4 of apolipoprotein E in patients with hypertensive cerebral hemorrhage was significantly higher than that in control group (P 〈 0,01), E3/3 was significantly lower than control group (P 〈 0.01 ). ② The allegomorph frequency in patients with hypertensive cerebral hemorrhage was the most in E3, followed by E4, and the least was E2 (40.83%, 37.92%, 21.25%). In control group the allegomorph frequency was the most in E3, followed by E2, and the least was E4 (76.67%, 15.00%, 8.33%). ③ Compared with control group, the frequency of allegomorph E4 in patients with hypertensive cerebral hemorrhage was significantly higher, the frequency of E3 was significantly lower (P 〈 0.01 ). CONCLUSION: The frequency of E4 in patients with hypertensive cerebral hemorrhage was significantly higher than that in control group, and the frequency of E3 was significantly lower than that in control group, which indicates that E4 is a risk factor in youth patients with hypertensive cerebral hemorrhage and E3 may has some protective effect.
