不同吸痰方式对急性呼吸衰竭患者呼吸系统顺应性的影响

The effect of different modes of sputum aspiration on respiratory compliance in acute respiratory failure patients

目的研究不同吸痰方式对急性呼吸衰竭患者呼吸系统顺应性的影响。方法选择21例急性呼吸衰竭并行机械通气治疗的患者,随机分为两组,A组(n=11):先进行密闭式吸痰,后施行开放式吸痰;B组(n=10):先进行开放式吸痰,后施行密闭式吸痰。观察记录两组患者行不同方式吸痰后2min呼吸系统力学参数的变化。结果两种吸痰方式均不影响呼吸系统顺应性(P>0.05),但开放式吸痰可致气道压力升高(P<0.05)。结论不同吸痰方式对急性呼吸衰竭机械通气患者呼吸系统顺应性无影响;密闭式吸痰有利于维持良好的气道压力,可作为急性呼吸衰竭机械通气患者进行气管内吸痰时的首选方法。

Objective This study aimed to the effect of different modes of sputum aspiration on respiratory compliance in acute respiratory {allure patients. Methods 21 acute respiratory failure patients, received mechanical ventilator, were divided into two groups. In A group （n= 11）, the patient received closed aspiration firstly, and then received opened aspiration. In B group, the patient received opened aspiration firstly, and then received closed aspiration. The respiratory parametric changes were recorded 2 minutes after aspiration finished. Results The two different modes of aspiration had no effect on respiratory compliance（P〉0.05）. But the opened modes resulted in an increased airway pressure（P〈0.05）. Conclusion The different aspiration modes had no effect on respiratory compliance. The dosed aspiration system could be the first choice for airway management, because it administered to keeping the normal airway pressure.
