环氧化酶-2蛋白在膀胱移行细胞癌中的表达及意义

EXPRESSION OF CYCLOOXYGENASE-2 IN TRANSITIONAL CELL CARCINOMA OF THE BLADDER AND ITS SIGNIFICANCE

①目的探讨环氧化酶-2(COX-2)蛋白在膀胱移行细胞癌(TCCB)中的表达及其与肿瘤生物学行为之间的关系。②方法采用免疫组化SP法检测120例膀胱移行细胞癌和10例正常膀胱黏膜标本COX-2蛋白的表达。③结果正常膀胱黏膜COX-2呈阴性表达,在膀胱移行细胞癌G1、G2、G3级和Tis^T1、T2~T4期阳性表达率分别为40.00%、61.02%、93.75%和44.83%、90.91%,COX-2的阳性率在不同组织学分级的TCCB中差异有极显著性(χ2=13.96,P<0.01),在不同临床分期的TCCB中差异有极显著性(χ2=20.70,P<0.01),在复发组与未复发组COX-2阳性率分别为82.50%和45.00%,差异有极显著性(χ2=15.35,P<0.01)。④结论COX-2阳性表达与膀胱移行细胞癌病理分级、浸润及复发关系密切,COX-2可以作为判定膀胱移行细胞癌恶性程度及复发的参考指标。

Objective To study the expression of cyclooxygenase-2 （COX-2） in transitional cell carcinoma of the bladder （TCCB） and analyze its correlation with the biological behavior of the tumor. Methods The expressions of COX-2 in 120 cases of TCCB and 10 cases of normal bladder Tis sue were detected by streptavidin-peroxidase immunohistochemical method. Results COX-2 was negatively expressed in normal bladder mucosa. The positive expression rates of COX-2 were 40.00%, 61.02% and 93.75% for G1, G2 and G3, 44.83% and 90.91% for Tis-T1 and T2-T4 tumors, respectively. The positive expressions in three groups with different histological grading in TCCB showed significant difference （x^2=13.96, P〈0.01）, and the difference showed great significance as well in two different clinical staging （x^2=20.70,P〈0.01）. The expression of COX-2 in recurrence group and non recurrence group was 82.50% and 45.00%, respectively, and the difference was significant （x^2=15.35, P〈0.01）. Conclusion The positive expression of COX-2 in TCCB is closely related to the tumor-cell grade, depth of invasion and recurrence. The expression of COX-2 may be used as a parameter for assessing the malignancy and recurrence of TCCB.
