新生儿巨细胞病毒感染与胆道闭锁肝脏纤维化的相关研究 被引量:28

Relationship between infection of cytomegalovirus and liver fibrosis in biliary atresia

摘要 目的探讨新生儿巨细胞病毒感染与胆道闭锁肝脏损伤的关系.方法回顾我院2004年1月~2005年1月收治21例胆道闭锁患儿临床资料.对肝组织纤维化和肝细胞变性坏死程度分级,同时进行肝脏和肝门纤维块巨细胞病毒-pp65免疫荧光染色.根据血清及病毒学检查结果将病人分组,比较两组患儿肝功能,肝脏纤维化,肝细胞破坏程度.结果巨细胞病毒感染组13例,非感染组8例.两组肝功能各指标除总胆红素,γ-谷氨酰转肽酶外无明显差异.巨细胞病毒感染组肝脏纤维化程度重于非感染组(P<0.05).结论巨细胞病毒活动性感染加重胆道闭锁患儿胆汁淤积和肝脏的纤维化. Objective To investigate the relationship cytomegalovirus (CMV) and liver damage in biliary atresia with BA in our hospital were reviewed. The patients were between prenatal or perinatal infection of (BA). Methods The data from 21 infants classified into CMV infection group (n = 13) and non-infection group (n = 8) based on the results of serologic investigations and virus detections. To detect the virus, cytomegalovirus-pp65 immunofluorescence staining was performed on liver samples and extrahepatic biliary remnants in these patients. The degrees of liver fibrosis and degeneration of hepatic cells were evaluated by the histological examination. The liver function, the degrees of liver fibrosis and the levels of degeneration of hepatic cells were compared between these two groups. Results Significant differences were found in the levels of total bilirubin andT-glutamyl transpeptidase between two groups. The liver fibrosis was more significant in infection group than that in the non-infection group (P〈0. 05). Conclusions Our results show that the infection of cytomegalovirus exacerbates liver fibrosis and cholestasis in BA.
