外侧入路微创全髋置换术

Minimally Invasive Total Hip Arthroplasty through Lateral Incision

目的:探讨微创下行人工全髋关节置换(THA)的手术适应证、技术要点.方法:采用外侧小切口(8～10cm)施行THA手术(微创THA组)47例(共50髋);同期按常规方法行THA手术(常规THA组)41例(41髋).对两组围手术期出血量、切口引流量、输血量、关节置换时间等指标进行比较.结果:微创THA组在围手术期出血量、术后切口引流量、输血量等均较常规THA组少,术后1周可扶双拐下地不负重行走,术后2周可扶单拐不负重行走,术后弃拐时间同常规THA组.结论:微创THA术后恢复快、围手术期出血少、软组织创伤小、关节置换时间短、假体功能良好,但应注意手术适应证及技术熟练程度.

Objective： To explore the indication in minimally invasive surgery（MIS） total hip arthroplasty（THA）for hip disorders. Methods： Forty-seven patients（50 hips）were performed with MIS THA, THA was performed with standard THA at the same period. The comparative items included perioperative bleeding, operative time, incision length, posto perative functional recovery and prosthetic position. Results： The length of incision was 9.1 cm（ranged from 8.5 cm to 9.7 cm）in MIS THA group and 17.2 cm（ranged from 15 to 22 cm）in standard THA group（P 〈 0.01）. Conclusion： The advantages of MIS THA are less traumatic tissue, less blood loss, good cosmetic incision and rapid recovery of function.
