兔膝关节软骨缺损对关节退变的影响 被引量:12

摘要 目的:探讨软骨损伤与关节退变之间的关系,以及持续被动运动治疗在其中的作用。方法:新西兰大白兔15只共30个膝关节随机分为3组,采用在关节软骨上钻孔的方法建立软骨损伤模型,术后按照不同的处理分为假手术组、对照组和持续被动运动(CPM)组,各组采取相应的处理方法。术后12周处死实验兔并进行关节软骨大体评分、HE染色评分和MMP-3免疫组化染色。结果:A组软骨表现正常。B组软骨在肉眼观察、病理学表现和MMP-3阳性细胞分数改变上均显示有明显退变。C组的各项指标均与A组接近而与B组差异有显著性意义(P<0.05)。结论:人为造成兔膝关节软骨缺损,可以进展成为骨关节炎(OA)。软骨缺损后早期给予CPM治疗8h/d,连续4周,在12周后可有效减缓OA的发生。 Objective: To discuss the relationship between cartilage damage and eartilage degeneration and the effect of eontinuous passive motion on it. Method: 15 New Zealand rabbits with 30 knee cartilage are divided into 3 groups randomly. The cartilage damage model was established by drilling on the cartilage. The rabbits that had been operated on were subjected to three forms: group A with fake operation, control group (B) and group C with eontinuous passive motion. Each group has the corresponding pnst-operative management. The experimental rabbits were killed at 12th week after operation and were assessed by the gross appearances, hematoxylin and eosion stain and MMP-3 immunohistochemistry. Result:The cartilage of group A showed normal structure, group B showed obviously degeneration which can be observed in gross appearances, the score of pathology and the ration of MMP-3 positive cells. The targets of group C get close to group A. But they have the obvious difference(P〈0.05) compared with group B. Conclusion:Man-made defects in the knee articular cartilage in rabbits may develop into osteoarthritis.The early use of continuous passive motion which was used 8h a day and last 4 weeks after cartilage defect can avoid the happen of osteoarthritis in 12 weeks.
