糖尿病与胆石症临床关系初探

Study on Relationship Between Diabetes and Cholelithiasis

目的探讨糖尿病与胆石症之间的发病关系。方法对120例糖尿病患者进行CT、B超等项检查。结果120例糖尿病患者中伴有胆石症36例,胆管结石2例,共38例,占31.6%,其中19例无临床症状,占50%,18例有不同程度的血脂增高,占47%。结论糖尿病患者胆石症发生率高,且年龄越大,发病率越高,危险性越大。糖尿病患者脂质代谢紊乱造成的高脂血症,自主神经病变造成的胆囊收缩功能低下及免疫力低下所致的胆囊炎症发生,可能是形成胆石症的发病机理。

Objective To investigate the pathogenic relation between diabetes and cholelithiasis. Methods CT and Ultrasonic examinations were taken in 120 cases with diabetes. Results In 120 cases, there were 36 cases with cholelithiasis and 2 cases with biliary duct＇s lithiasis, account for 31.6 %, in which 19 cases（50 % ） with no clinical symptoms and 18 cases（47 % ） with different extent hyperlipemia. Conclusions The incidence rate of cholelithiasis was high in patients of diabetes. The incidence rate and risk increased with the age. The pathogenic mechanism of cholelithiasis is possibly due to the hyperlipemia resulted from the lipid metabolic disturbance in, the lower contractibility of gallbladder resulted from the autonomic nervous pathologic change, and the cholecystitis resulted from the hypoirnmunity.
