BiPAP通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭

Observation on the effects of the BiPAP ventilation's curing COPD with Tape Ⅱ respiratory failure

目的探讨BiPAP治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的疗效。方法28例COPD合并Ⅱ型呼吸衰竭患者随机分为BiPAPA通气加常规治疗(治疗组)和单用常规治疗(对照组)各14例,定期记录生命体征、血气分析变化,观察两组的治疗效果。结果通气2h两组比较治疗组的PaO2明显增加、心率和呼吸频率降低(P<0.05);12h治疗组PaCO2显著下降、pH值改善(P<0.05);需气管插管者,对照组14例有10例(71.42%),而治疗组14例有3例(20.42%),有显著减少(P<0.05)。结论BiPAP通气可作为COPD并呼吸衰竭的一线治疗手段。

Objective To evaluate the effects of the mask BiPAP ventilation on COPD with Tape Ⅱ respiratory failure. Methods Thirty cases of COPD with Tape Ⅱ respiratory failure were randomly divided into two groups： BiPAP ventilation together with regular therapy group （ test group, n = 14） and regular therapy group （contrast group, n = 14）. The patients condition, blood and vital energy parameter were recorded. Results In the test group when ventilated for 2h, PaCO2 increased and heart rates and respiratory rates decreased （P〈0.05）. When ventilated for 12h, PaCO2 decreased and acidos improved （P〈0.05）. In the contrast group, there were 10 cases （71.42%） who needed endotracheal intubation,while in the test group were 3 cases（20.42%） （P〈0.05）. Conclusion BiPAP ventilation can be used as the first choice for the treatment of COPD with Tape Ⅱ respiratory failure.
