Forsus矫治器治疗安氏Ⅱ类远中错（牙合）的临床应用

Treatment of Class Ⅱ malocclusion with Forsus appliance

目的本研究的目的在于探讨Forsus矫治器治疗安氏Ⅱ类远中错(牙合)患者牙性、骨性所发生的变化,并掌握一种新矫治器的临床操作常规.方法临床选择11例安氏Ⅱ类远中错(牙合)患者,年龄1215岁,恒牙(牙合),采用Forsus矫治器,对矫治前后的X线片进行头影测量分析.结果 (1)11例患者经过平均5.59个月的治疗均获得Ⅰ类磨牙关系,前牙覆盖从平均9.25mm减至2.76mm,覆(牙合)正常,上下牙列排齐,面型改善明显.(2)矫治前后X线头影测量显示:SNA基本未变,SNB增加2.29°,ANB角减小2.26°,差异有显著性,从而使上下颌间关系更为协调;上切牙与NA角从31.65°减小至16.18°,距离从8.08mm减小至3.02mm,差异极具显著性,上切牙内收明显;下切牙与NB角从28.48°增加至32.91°,距离从6.45mm增加至6.74mm,说明矫治中下切牙受到近中向的力,少许唇倾.结论 Forsus矫治器结构简单,临床操作方便快捷,可与固定矫治器同时使用,对于恒牙早期Ⅱ类下颌后缩患者矫治效果明显,对现有的安氏Ⅱ类矫治系统是一个很好的补充.

Objective To investigate the skeletal and dental changes of Class Ⅱ patients treated with Forsus appliance. Methods Eleven ClassⅡ patients（age range 12-15 years） with retrusive mandible were treated with Forsus appliance in the early permanent dentition. Lateral cephalometric films were taken at the beginning and the end of the treatment. Cephalometric analysis was performed. Results （1） Class Ⅰ occlusal relationship was achieved in all patients in an average treatment time of 5.59 months. Overjet decreased from 9. 25 mm to 2.76 mm and the profile of patients was improved. （2） The skeletal changes consisted of the increase in SNB（by 2.29 degree） and decrease in ANB（by 2.26 degree）. Dental alterations comprised retrusion of the upper incisors and labial tipping of the lower incisors. Conclusions Forsus appliance was effective to correct Class Ⅱ malocclusion in growing patients.
