膝骨性关节炎患者生存质量及其证候分布关系分析

AN INVESTIGATION ON RELATIONSHIP BETWEEN QUALITY OF LIFE AND TCM SYNDROMES IN DEGENERATIVE KNEE OSTEOARTHRITIS

为了解膝骨性关节炎(膝OA)患者生存质量及其证候分布关系,对204例膝退行性骨关节病患者进行一般情况、生存质量流行病学调查。结果膝OA患者的生存质量总评分得分较低,各维度受损程度按得分由大到小依次为躯体维度、心理维度、社会维度、物质维度;各证候患者生存质量均有不同损害,血瘀证的生存质量总分量低;生存质量与证候之间关系示瘀血证、寒痹证对QOL的影响有统计意义(P<0.05),瘀血证除物质维度影响无统计意义外,其余各方面影响均有统计意义,且均呈负相关关系;寒痹证只在物质维度有显著影响,余方面皆无统计意义。QOL各因子得分对血瘀证的影响分析结果示住房、躯体不适感、运动功能、正性情感和婚姻家庭等因子对血瘀证的发生影响有统计意义。其余各证候对QOL的影响均无统计意义。

In order to explore the relationship between quality of life (QOL) and TCM syndromes of degenerative knee osteoarthritis (KOA), epidemic investigations were made on the general conditions and QOL of 204 cases of KOA. The results showed that the total QOL scores of KOA patients were under the normal; all four dimensions of QOL were impaired with the body dimension, psychological dimension, social dimension and substantial dimension arranged in the injury severity order; the QOL of the patients of all syndromes was impaired in varied degrees with the blood-stasis syndrome having the lowest total QOL score; the relationship between QOL and TCM syndromes showed that only blood-stasis and cold-arthralgia syndromes among all the syndromes influenced QOL with a statistical significance (P < 0.05); the blood-stasis syndrome had a statistically-significant influence on all the dimensions except the substantial one, being negatively correlated; the cold-arthralgia-syndrome had a significant influence only on the substantial dimension and on others with no statistical significance; many factors, such as housing, physical malaise, movement function, positive affection, marriage, family, etc may influence the blood-stasis syndrome with a statistical significance but other TCM syndromes influence QOL with no statistical significance.
