左侧梗阻性结肠癌一期切除吻合及围手术期处理

Primary resection and anastomosis and perioperative management of the carcinoma of left colon with obstruction

目的探讨左侧梗阻性结肠癌Ⅰ期切除吻合的可行性和恰当的围手术期处理。方法对左侧结肠癌致急性肠梗阻26例患者采用Ⅰ期切除吻合的临床资料进行回顾性分析。结果本组无吻合口瘘、腹腔感染等并发症发生。切口不同程度液化感染8例,无围手术期死亡。结论对于能够耐受手术切除的左侧梗阻性结肠癌患者,Ⅰ期切除吻合是完全可行的。术中彻底的肠道减压和准确的吻合方式是减少术后并发症的关键。

Objective To explore the clinical efficacy of the primary resection and anastomosis and rational perioperative management of the carcinoma of left colon with obstruction. Method The chnical data of 26 cases were studied retrospectively. Results Incision wound infection and liquefaction occurred in 8 cases. No patient had severe complications such as fistula at anastomosis and infection of abdominal cavity. No patient died in the perioperative period. Conclusions The primary resection and anastomosis for treatment of the carcinoma of left colon with obstruction is practicable if the patients can bear it. Effective colonic decompression and accurate anastomosis are the important factors for decreasing postoperative complications.
