摘要 目的:分析HLA-Ⅰ类血清学分型误定规律,研究HLA-A,B位点基因分型方法,提高移植配型的准确性。方法:应用聚合酶链反应—序列特异性引物(PCR-SSP)技术,检测180例骨髓资料库中已用血清学分型的自愿者HLA-A,B基因。结果:HLA-A特异性血清学分型错误率在纯合子与杂合子中分别占30.77%与17.19%,HLA-A特异性血清学分型总错误率19.48%;HLA-B特异性血清学分型错误率在纯合子与杂合子中分别占46.15%与30.14%,HLA-B特异性血清学分型总错误率32.09%;HLA-A,B血清学分型总错误率为27.50%。HLA-A特异性血清学分型假阴性8.75%,假阳性2.27%,错误指认特异性17.21%,HLA-B特异性则分别为9.00%、2.18%、29.91%。结论:HLA-A和HLA-B纯合子血清学分型错误率明显高于对应的杂合子分型(P值均小于0.005),HLA-B特异性血清学分型错误率显著高于HLA-A特异性;骨髓资料库中自愿者HLA-A,B位点须重新用基因分型方法分型。 Objective: To analyze the rule of HLA-Ⅰ serological mistyping, to investigate on ganotyping of HLA-A,B loci, and to improve the accuracy of transplant match.Methods:HLA-A, B alleles from 180 bone volunteer donators in Bone Register, whose HLA-A, B serological specifics were typed, were genotyped with PCR-SSP.Results:The error rate in HLA- A serological typing was 30.77% in the homozysotes and 17.19% in the heterozygotes (total 19.48%), and that was 46.15% in the homozygotes and 30.14% in the heterozygotes (total 32.09%) in the HLA-B serological typing; Total error rate of HLA-A, B serological typing was 27.50%; False positive, false negative and misaasignment of the HLA-A serological typing were 8.75% ,2.27% and 17.21% respectively, and that of the HLA-B serological typing were 9.00%, 2.18% and 29.91% respectively.Conclusion:The error rate of HLA- A and HLA- B serological typing in the homozysotes was significantly higher than that in the respective heterozysotes (P〈0.005); Error rate of HA-B serological typing was high- er than that of HLA-A; HLA-A, B PCB-SSP typing was more accurate than serological typing and suitable for clinical application.In order to improve accuracy and extend the lives of transplanting patients, HLA-A, B alleles of bone volunteer donators in the Bone Register must be ganotyped.
