VEGF和上皮钙黏蛋白在膀胱移行细胞癌组织表达 被引量:5

EXPRESSIONS OF VASCULAR EN DOTHELIAL GROWTH FACTOR AND E-cadherin IN TRANSITIONAL CELL CARCINOMA OF THE BL ADDER

摘要 ①目的探讨血管内皮生长因子(VEGF)和上皮钙黏蛋白(E-cadherin)在膀胱移行细胞癌(TCCB)中的表达及其与肿瘤生物学行为之间的关系。②方法采用免疫组化SP法检测68例TCCB和10例正常膀胱黏膜标本VEGF和E-cadherin的表达。③结果正常黏膜VEGF不表达,在TCCBⅠ级(G1)、Ⅱ级(G2)、Ⅲ级(G3)和浅表性(Tis^T1)、浸润性(T2~T4)肿瘤的阳性表达率分别为23.07%、58.62%、84.61%和43.75%、77.77%;VEGF的阳性率在不同组织学分级及不同临床分期的TCCB中差异有极显著性(χ2=14.11、8.29,P<0.01);在复发组与未复发组的阳性表达率分别为78.94%和40.00%,差异有极显著性(2χ=10.77,P<0.01)。正常黏膜E-cadherin呈阳性表达,在TCCBG1、G2、G3和Tis^T1、T2~T4肿瘤的异常表达率分别为15.38%、55.17%、80.77%和37.50%、75.00%;E-cadherin的异常表达率在不同组织学分级及不同临床分期的TCCB中差异有极显著性(χ2=15.27,P<0.01);在复发组与未复发组的阳性表达率分别为73.68%和36.67%,差异有极显著性(χ2=9.40,P<0.01)。④结论VEGF阳性表达和E-cadherin异常表达与TCCB病理分级、浸润及复发关系密切,VEGF和E-cadherin可以作为判定TCCB恶性程度及复发的参考指标。 Objective To study the expressions of vascular endothelial growth factor(VEGF) and E-cadherin in transitional cell carcinoma of the bladder (TCCB) and analyze their correlation with the biological behavior of the tumor. Methods The expressions of VEGF and E-cadherin in 68 cases of TCCB and 10 cases of normal bladder tissue were detected by streptavidin-peroxidase immunohistochemical method. Results VEGF was negatively expressed in normal bladder mucosa. The positive expression rates of VEGF were 23.07%, 58.62% and 84.61% for G1, G2 and G3, 43.75% and 77.77% for Tis-T1 and T2 -T4 tumors, respectively. The positive expression rate in three groups with different histological grading in TCCB showed significant difference(χ^2= 14.11, P〈0.01), and the difference showed great significance as well in two different clinical staging (χ^2 = 8. 29, P〈0.01). The expression of VEGF in recurrence group and non-recurrence group was 78.94% and 40. 00%, respectively, and the difference was significant (χ^2 = 10.77,P〈0.01). E-cadherin was positively expressed in normal bladder mucosa. The abnormal expression rates of E-cadherin were 15.38% ,55.17% and 80.77% for G1, G2 and G3, 37.50% and 75.00% for Tis-T1 and T2 - T4 tumors, respectively. The abnormal expression rate in three groups with different histological grading in TCCB showed significant difference (χ^2=15.27,P〈0.01), and the difference showed great significance as well in two different clinical staging (χ^2= 9.73,P〈0. 01). The abnormal expression of E-cadherin in recurrence group and unrecurrence group was 73.68% and 36.67%, respectively, and the difference was significant (χ^2 =9.40,P〈0.01). Conclusion The positive expression of VEGF and abnormal expression of E-cadherin are closely related to the tumor-cell grade, depth of invasion and recurrence. Both the expressions of VEGF and E-cadherin may be used as parameters for assessing the malignancy and recurrence of TCCB.
