摘要 目的研究后腹腔镜手术与腹腔镜手术中机体对CO2气体吸收的不同。方法腹腔镜手术患者30例,选择胆囊切除术28例、精索静脉高位结扎2例;后腹腔镜手术患者30例,选择肾囊肿去顶减压术。根据公式计算出不同手术时段机体对CO2的排除量。结果在手术的第1个15min和第2个15min内,两组患者的CO2排除量无明显差异,而在第3个15min,后腹腔镜组患者CO2排除量及排除增加量明显高于腹腔镜组。结论经由后腹腔的腹腔镜手术比经由腹腔的腹腔镜手术引起更为明显的高CO2血症,在进行长时间后腹腔镜手术操作时,应采取措施减少CO2的吸收。 Purpose To study the difference of CO2 absorption between retroperitoneoscopy and laparoscopy. Methods Sixty patients were equally divided into two groups,laparoscopy group and retroperitoneoscopy group, for comparison between them. The patients of laparoscopy group underwent cholecystectomy and high ligation of spermatic vein and those of retroperitoneoscopy group underwent renal cyst truncation. The depletion of CO2 in different time of operation was calculated by formula. Results There was no difference between two groups in CO2 depletion in the first and the second 15 minutes. The CO2 depletion increased obviously in retroperitoneoscopy group in the third 15 minutes. Conclusions The use of retroperitoneal path will cause obvious hypercapnia in patients than of peritoneal path. Something should be done to reduce the absorption of CO2 in the protracted retroperitoneoscopy.
