摘要
目的探讨99mTc-TRODAT-1SPECT脑显像对帕金森病(PD)患者诊断和病情监测的临床应用价值。方法选择29例PD患者(病例组)和17例非PD患者(对照组),行99mTc-TRODAT-1SPECT检查;选纹状体显像较为清晰的层面进行半定量分析,利用ROI技术分别计算纹状体/枕叶(ST/OC)和纹状体/额叶(ST/FC)放射性计数比值代表DAT功能水平;以SPSS10.0软件包行t检验和回归分析。结果病例组放射性计数比值显著低于对照组(P<0.01),H-YI期患者症状同侧放射性计数比值显著低于对照组(P<0.05),症状对侧(右侧)ST/OC比值与UPDRS成负相关(r=-0.0118,P=0.04)。结论99mTc-TRODAT-1SPECT显像可作为临床诊断PD的一种比较客观的检查方法,以便早期诊断和监测PD。
Objective To evaluate the clinical value of ^99mTc-TRODAT-1 SPECT imaging for diagnosing Parkinson's disease (PD) and monitoring disease progress. Methods Twenty-nine PD patients (PD group) and seventeen non-PD patients (control group) were examined with ^99mTc-TRODAT-1 SPECT. The clearer slices of images were chosen for semi-quantitative analysis. Regions of interest (ROIs) on the striatum were drawn to calculate the radioligand binding ratio. The level of DAT was expressed as the striatum/occipital (ST/OC) and striatum/frontal (ST/ FC) ratio. The Student t test and regression analysis were performed by using SPSS 10.0 software for Windows. Results A significant reduction of mean radioligand binding ratios was found in PD group compared with control group (P 〈0.01 ). A significant reduction of mean radioligand binding ratios ipsilateral to the symptoms was found in the Hoehn-Yahr stage Ⅰ group compared with control group (P 〈0.05). There was a negative regression between the mean ST/OC ratios contralateral to the symptoms ( or right) and UPDRS scores ( r = - 0. 0118, P = 0.04). Conclusion ^99mTc-TRODAT-1 SPECT image can serve as an objective examination for clinical diagnosis of PD. It has positive clinical value in early diagnosing and monitoring disease progress.
