白内障手术致角膜后弹力层脱离的原因及处理

Reasons and treatments of corneal descemet membrane detachment during cataract operation.

目的分析白内障手术致角膜后弹力层脱离的原因,提出预防及处理方法。方法对498例(512眼)白内障施行小切口非乳化白内障囊外摘出人工晶状体植入术,术中30例(30眼)各种原因造成角膜后弹力层脱离的对症处理。结果分析白内障手术中由于切口的方式、宽度、位置及手术器械的机械性损伤等原因,造成角膜后弹力层不同程度的脱离。结论术中及早发现和及时处理角膜后弹力层脱离,对预防术后的并发症及视力的恢复均起到重要作用。

Objective To analyze the reasons of corneal descemet membrane detachment during cataract operation and find the treatment methods. Methods Small incision extracapsular cataract extraction and IOL implantation was performed on 498 patients （512 eyes） of cataract, descemet membrane detached in 30 patients （30 eyes）. Results The factors of corneal descemet membrane detachment during cataract operation included incision type, length, location, mechanic injury and so on. Conclusion Early diagnosis and treatment are important for preventing postoperative complication.
