脊柱转移癌手术疗效探讨

Preliminary results of surgical treatment of metastatic spine disease

目的观察手术治疗脊柱转移癌的疗效。方法对16例患者施行肿瘤切除重建手术,术前、术后神经功能按Frankel分级评估;生活质量状况按(edmontonsymptomassessmentscale,ESAS)评估,并进行统计学分析。结果随访3￣16个月,10例神经功能恢复按Frankel分级除1例外,提高1～3级;术后疼痛缓解最为明显,其次依次改善的有疲乏、恶心、精神状况、焦虑和食欲症状。结论选择适宜病人,行脊柱转移癌切除重建手术,可以明显的改善神经功能和生活质量状况。

[Objective] To object the efficacy of surgical treatment of metastatic spine disease. [Methods] Sixteen patients under going surgery for spinal metastases were prospectively evaluated. Pre-and postoperative assessments were performed using the Edmonton sympton Assessment scale （ESAS） for Overall quality of life and Franke] Funtional Assessment for spinal cord function. Paired “t” tests were used to assess the significance of the difference between pre-and postoperative ESAS scores. [Results] The average length of follow-up was 8.4 months （rang 3-16 months）. Among the 10 patients, 9 Patients gained one to three Frankel grade of nenrologie reeorery. Six months after surgery, The largest improvements was noted in the domain of pain, There were also statistically significant improvements in the domains of tiredness, nausea, anxiety, appetite, and overall well-being. [Conclusion] In the approprate patients, Surgical management bring about a postive effect on the overall qulity of life and neurologie function recovery by resection of spinal tumor and reconstruction of spinal stability.
