膀胱腺癌38例报告

Adeno-carcinoma of urinary bladder(Report of 38 cases)

１９８０年８月～１９９３年８月收治膀胱腺癌３８例，其中原发性膀胱腺癌１２例，脐尿管膀胱腺癌６例，继发性膀胱腺癌２０例。本组３４例获得随访１～１０年，根治性膀胱全切除１０例，存活９例，膀胱部分切除１９例，存活４例，放疗或激素治疗９例，无一例存活，多数１年内死于肿瘤转移，５年生存率为１７．６４％。主张手术应采用根治性膀胱全切除。对膀胱腺癌的组织来源、发病状况、治疗及预后等进行了讨论。

Adenocarcinoma of the urinary bladder has been diagnosed histologicaly over a 14-year period in 38 cases,being primary in 12,secondary to urachal adenocarcinoma in 6 and secondary to prostate,colon or ovary in 20.34 patients have been followed up for 1 to 10 years. Among which,10 had undergone radical total cystectomy and 9 being alive and well.Only 4 of the 19 patients undergone partial cystectomy have been well and alive. All the 9 patients managed by radio or hormonal therapy alone died within one year.The total 5-year survival rate has been 17.64%. Radical total cystectomy seems to be the best for adenocarcinoma of the urinary bladder.The histogenesis,incidence,treatment and prognosis of adenocarcinoma of urinary bladder were reviewed.
