肾癌患者手术前后外周血淋巴细胞CD25、Ki67表达的变化及意义

Changes of pheripheral blood lymphocytes CD25 and Ki67 in renal cell carcinoma preoperation and postoperation and the significance

目的探讨手术对肾癌患者外周血淋巴细胞CD25、Ki67表达的影响。方法应用流式细胞术检测16例正常人和32例肾癌患者外周血淋巴细胞CD25、Ki67的表达。结果肾癌患者手术后外周血淋巴细胞CD25、Ki67的表达分别为(21.37%±4.66%,21.92%±16.08%)均高于手术前(20.30%±6.23%,17.05%±9.87%),但没有统计学意义(P=0.397,0.076)。无论术前、术后,肾癌患者外周血淋巴细胞CD25、Ki67的表达均显著高于正常对照组(P<0.01)。10例患者(31.2%)术后外周血淋巴细胞CD25、Ki67的表达同时升高,2例患者(6.3%)术后外周血淋巴细胞CD25、Ki67的表达同时降低,20例患者(62.5%)术后外周血淋巴CD25、Ki67的表达变化方向不一致。结论肾癌患者手术前后外周血淋巴细胞CD25、Ki67表达的变化,可能提示机体自身的免疫状况和免疫“应激”能力。尚须进一步的研究资料来了解肾癌患者术后更长时间外周血淋巴细胞CD25、Ki67表达的变化。

Objective To investigate the changes and the significance of peripheral blood lymphocytes CD25 and Ki67 before and after operation in 32 patients with renal cell carcinoma who underwent radical nephrectomy. Methods Peripheral blood lymphocytes CD25 and Ki67 were detected by flowcytometry in 32 patients with RCC and 16 healthy individuals served as controls. Results The expressions of CD25 and Ki67 postoperation were higher than those preoperation （21.37%±4.66%, 21.92%±16.08%; 20.30%±6.23%, 17.05%±9.87%）, but with no statistical significance. The expressions of peripheral blood lymphocytes CD25 and Ki67 were greatly higher than those in the controls （P〈0.01）. The expression of peripheral blood lymphocytes CD25 and Ki67 postoperation elevated in 10 cases（31.2%） and decreased in 2 cases（6.3%） simultaneously. Changes of the expressions of peripheral blood lymphocytes CD25 and Ki67 postoperation were inconsistent in 20 cases （62.5%）. Conclusion Changes of pheripheral blood lymphocytes CD25 and Ki67 in renal cell carcinoma may implicate immune status and ability “for fight or flight” in patients. This concept may be important for future immunotherapy of RCC. More postoperative changes of pheripheral blood lymphocytes CD25 and Ki67 in RCC in longer duration remain unknown. Further investigations are still required to be performed.
