甲状腺微小癌的外科治疗

Surgical therapy of thyroid microcarcinoma

回顾性分析1994年1月～2004年6月经我院收治及病理证实的甲状腺乳头状微小癌82例患者的临床资料.82例乳头状微小癌中多灶癌8例(9.8%)颈淋巴结转移14例(17.1%).随访80例,时间1～10年,平均6年,随访中同侧颈部淋巴结转移8例而行二次颈清除术,无局部复发,无死亡,未发现远处转移.术中探查及冷冻切片对提高甲状腺乳头状微小癌的诊断非常重要,并且可以根据其生物学行为不同而选择不同的切除范围,疗效肯定.

To investigate the biological behavior.clinical characteristics and the resection extent thyroid microcarcinoma, the clinical data of 82 cases with papillary thyroid microcarcinoma and confirmed by pathology from 1994 to 2004 were retrospectively analyzed. Neck lymph node metastasis was found in 14 cases （ 17. 1 %）, muhicentric tumors were 8（9.8%）, All patients were followed up from 1 to 10（mean 6）years. Lymph node metastasis were found in 8 cases and neck lymph node dissection were performed again. No local recurrence and death. Exploratory operation and frozen section are very important factors to heighten diagnosis of thyroid microcarcinoma. We may select different resection extent according to biological behavior of thyroid microcarcinoma.
