摘要
目的:探讨近23年呼吸系统肿瘤发病趋势及病理特点,为病理医师和临床医师提供有价值的肿瘤病理资料.方法:选自天津医科大学总医院和第二医院病理科1981~2003年23年外科手术和活检病理档案中的呼吸系统肿瘤病例共5 785例,以MicrosoftExcel表格形式存储,应用SPSS11.5对该资料进行统计分析.结果:1)23年间恶性肿瘤总检出率显著高于良性肿瘤,u=52.68,P=0.000.2)恶性肿瘤的好发部位主要是肺其次是喉;良性肿瘤的好发部位主要是鼻其次是咽.3)恶性肿瘤的发病高峰年龄为50~60岁;而良性肿瘤的发病高峰年龄为40~50岁.4)恶性肿瘤各部位的主要组织学类型为鳞癌,良性肿瘤各部位的主要组织学类型为乳头状瘤.5)23年来肺部恶性肿瘤的检出率呈缓慢上升趋势,近几年急剧增加;鼻咽部恶性肿瘤检出率则呈缓慢下降趋势;喉部恶性肿瘤检出率在1981~1997年呈上升趋势,在1997~2003年呈下降趋势.结论:呼吸系统恶性肿瘤尤其是肺癌检出率呈上升趋势,做好防治工作至关重要.
OBJECTIVE: To study the pathological characteristics and tendency of incidence of respiratory system tumors during the past 23 years in Tianjin and to provide information for pathologists and clinicians. METHODS: The data of respiratory system tumors were collected from General Hospital and the Second Hospital of Tianjin Medical University. The data were stored in Excel and analyzed by spss 11.5. RESULTS: 1) The detection rate of malignant tumors was significantly higher than that of benign tumors, u = 52. 68, P= 0. 000. 2) The common sites of benign tumors were nose and pharynx, but the common sites of malignant tumors were lung and larynx. 3) Benign tumors were more in age of 40-50 years, and malignant tumors were more in age of 50- 60 years. 4) The commonest histological type of malignant tumors was squamous cell carcinoma, but the commonest type of benign tumors was papilloma. 5) The detection rate of malignant lung tumors had been keeping stable increase during the past 23 years and getting sharp increase in recent years, but the detection rate of malignant nasopharynx tumors had been keeping stable decrease during the past 23 years. The detection rate of malignant larynx tumors increased between 1981 and 1997. followed by a decrease between 1997 and 2003. CONCLUSIONS: The detection rate of malignant respiratory system tumors especially lung cancer is gradually increasing, therefore early prevention and treatment is critical to patients' prognosis.
