子宫颈癌放疗中盆腔剂量均匀分布的方法探讨 被引量:4

摘要 子宫颈癌放疗失败的原因之一是盆腔剂量分布不均匀,主要原因是子宫偏位或移位。由于子宫偏位及移位的发生率极高,治疗中又难以弥补这种缺点,直接影响其疗效及并发症的发生,因此如何达到盆腔剂量的合理分布是目前宫颈癌放疗中十分重要的课题,是提高疗效的关键。近年来采用高剂量率腔内后装治疗,使治疗时间从24小时减少至几分钟,为此纠正宫腔管偏位及移位。 Uneven pelyic dose distribution owing to the extremely high incidence of uterinedeviation,remains one of the factors leading to failure in the intracavitary radiotherapyfor cervical cancer. Recent years, the application of high dose rate after-loading tech-hique has rendered favourable condition to correct the uterine deviation by the applica- tors. A simple way to correct the deviation of applicator is introduoed in this paper. Inorder to keep the applicator in ideal position, we advocate an alternative use betweenuterine and vaginial applicators. the staithess eteel uterine tube is placed into theuterine cavity and corrected by packing and firihg method. From A. P. and lateral X-ray film of pelyis, the dose of A. B. can be calculated. In 50 cases studied, the dose dif-ferenee between two sides of the pelvis could be reduced to minimum after correctingthe deviation of uterine applicator.
