摘要 目的探讨阴道彩超(TVCDS)检测子宫血循环阻力指数(RI)鉴别宫内外早孕的可行性。方法应用TVCDS检测36例未破型宫外早孕和50例宫内早孕的子宫动脉、螺旋动脉和滋养层动脉血流的RI值。结果宫外早孕组子宫动脉平均RI值高于宫内早孕组(P<0.01)。宫外早孕组子宫螺旋动脉彩色多普勒的阳性率则明显低于宫内早孕组(P<0.01);宫外早孕组附件区滋养动脉检出率(83.3%),特异性100%。结论TVCDS和PD各项指标结合停经史及血HCG检测对宫内宫外早孕的诊断和鉴别诊断有重要的临床价值。 Objective To explore the value of transvaginal color doppler sonography (TVCDS) in testing the resistance index of the blood flow for uterine helicing artery for differential diagnosis of early intrauterine pregnancy and early extopic pregnancy. Methods TVCDS was used to assess the RI of the blood flow of uterine artery, helicine artery and trophoblast artery in 36 ectopic pregnancy cases and 50 intrauterinc pregnancy cases. Results The RI of the blood flow in the group of ectopic pregnancy was higher than in the group of intrauterine pregnancy (P〈0.01). The positive rate of CDFI of uterine helicine artery in the group of ectopic pregnaniy was significantly lower than in the group of intrauterine pregnancy (P〈0.01). Conclusion Conjugated with menolipsis history and β-HCG, the various TVCDS and PD indicators have important clinical value for the diagnosis and differential diagnosis of early intrauterine pregnancy and early extopic pregnancy.
