摘要
目的:评价北京地区老年人群幽门螺杆菌(Hpylori)感染的血清流行率及影响因素.方法:采用整群随机抽样的方法,问卷调查了北京不同地区(城区、郊区、山区)55岁及以上的老人2006例,应用免疫印迹法测定血清Hp-IgG抗体.结果:老年人群总的Hpylori感染率为83.4%;山区高于城区和郊区(91.1%vs82.6%,77.2%,P<0.01),重体力劳动者,低文化程度和有胃病史者Hpylori感染率高于脑力劳动、高文化程度和无胃病史者(84.9%vs77.9%,t=7.769,P<0.05;OR=0.694,t=4.198,P=0.040;89.3%vs83.0%,P<0.05);高蛋白饮食者Hpylori感染率低于素食者(81.3%vs88.4%,t=12.03,P<0.01)、经常喝茶者不喝茶者(OR=1.353,t=4.533,P<0.05),不同年龄段的老人Hprlori的感染也存在显著差异(60-65vs80岁以上,90.4%vs83.1%,P<0.01).结论:Hpylori在北京老年人群中存在着较高的血清流行率,并且与地区、职业、文化程度、饮食习惯及病史有关.
AIM: To assess the epidemic rate of blood serum and the influential factors of Helicobacterpylori infection in aged people in Beijing. METHODS: Two thousand and six people, who were from different areas of Beijing (urban, suburban and mountain area) and 55 or over 55 years old, were involved. The questionnaire was designed to acquire related information. The blood samples were collected and the serum Hpylori-lgG was detected by immunoblotting. RESULTS: The total rate of Hpyloriinfection in aged people was 83.4%. The infection rate in mountain area was significantly higher than those in urban and suburban area (91.1% vs 82.6%, 77.2%, both P〈0.01 ). The infection rates in the population of physical laborers, low-educated people and people with a history of gastric diseases were higher than those in the mental laborers, high-educated people and people without gastric disease history (84.9% vs 77.9%, t = 7.769, P〈0.05; OR = 0.694, t = 4.198, P = 0.040; 89.3% vs 83.0%, P〈0.05). The rate of Hpyloriinfection in the people with high-protein diet was markedly lower than that in the vegetarians (81.3% vs88.4%, t= 12.03, P〈0.01 ),and the rate in the people with tea drinking habit was also lower than that without tea drinking habit (OR = 1.353, t = 4.533, P〈O.05). Significant differences also existed between people with different age stages (60-65 vs over 80 years old: 90.4% vs 83.1%, P〈0.01 ). CONCLUSION: High epidemic rate of blood serum of H pylori exists in the aged people in Beijing, and it is related to the region, occupation, education degree, dietary habit and gastric disease history.
