重型颅脑损伤标准外伤大骨瓣术后二次脑损伤

Clinical analysis of the second brain insult in Severe craniocerebral injury that treated by standard trauma craniectomy

目的探讨重型颅脑损伤(SCCI)行标准外伤大骨瓣开颅术后合并二次脑损伤(SBI)的发病规律、诊断及治疗特点,以期提高SCCI的临床救治效果。方法对我科近4年收治的31例SCCI行标准外伤大骨瓣开颅术后合并SBI患者的临床资料进行回顾性分析。结果SCCI行标准外伤大骨瓣开颅术后合并SBI,发生时间多在伤后1-7天,伤后3天为高峰。结论SBI可以明显加重原发性脑损伤,使SCCI死残率大大提高,应当针对其发生机制积极展开临床研究,加强预防与综合治疗。

Objective To explore the Clinical features of the second brain insult in severe craniocerebral injury that treated by standard trauma craniectomy. Methods 31 patients with severe traumatic craniocerebral injury that treated by standard trauma craniectomy were retrospectively analyzed, Results The second brain insults after standard trauma craniectomy in severe craniocerebral injury were present more than 7 days after injury, and at 3 days after injury, the SBI was to the maximum, Conclusion SBI was related to the increased mortality and depraved prognosis in severe craniocerebral injury patients that treated by standard trauma craniectomy.
