膀胱白斑的超微病理学研究

The ultrastructure pathological investigate of vesical leukoplakia

目的探讨膀胱白斑的超微病理变化及其临床意义,为临床诊断治疗提供可靠依据。方法取52例膀胱白斑病灶组织,在电子显微镜下观察其超微病理学改变。同时取病灶旁正常膀胱黏膜12例作对照。结果膀胱白斑组织电子显微镜下改变显著,表面由多层鳞状上皮细胞组成,胞核较幼稚,核仁明显,胞浆内张力原纤维丰富,36例上皮细胞胞浆内可见糖原储积,14例糖原颗粒散在分布,上皮细胞之间的间隙较宽,细胞表面均有丰富的指状突起,相邻细胞以指状突起相连,连接部位可见桥粒结构。上皮基底部形成乳头状结构。结论电子显微镜检可更加清晰的显示膀胱白斑的超微病理变化,且可提示病变程度、发病趋势、细胞增殖活跃程度等,是膀胱白斑的重要诊断依据。

Objective Discuss the Ultrastructure;Pathological change and the clinical meaning of vesical leukoplakia, to offer credible basis for clinical diagnose and treatment. Method By way of taking some tissue from the vesical leukoplakia focus for biopsy in 52 cases, and then observe their pathological change under the electron microscopy, simultaneously, take some normal veslcal mucous embrane for contraposing in the 12 cases. Results Remarkable change are found under the electron microcopy, the surface of vesical leukoplakia tissue is made up of multilayer squamous epithelium, with infantile nucleus whose nucleoli are obvious and tensile force fibril in cytoplasm is abundant. In 36 case plenty of hepatin grain can be found in cytoplasm. While in 14 cases, the hepatin grain particles distributed dispersedly. The space between epithelia is wider thannormal, with plenty of finger-like protuberances on the surface, which help connecting the contiguous cells. The connection sites can find abundance intercellular bridges. The bottom of epithelium form nipple tissue. Conclusion Electron microscopy can explicitly demonstrate the Oltrastructure pathological change of vesical leukoplakia, as well as indicate the degree of lesion, occurrence tendency and the degree of cytoplasia activity, which is an important diagnosis evidence.
