玻璃体手术治疗后段眼内异物及其创伤性视网膜裂孔

Vitrectomy for posterior segment intraocular fireign bodies associated traumic retinal tears

目的分析现代玻璃体手术治疗眼后段眼内异物及其创伤性视网膜裂孔和临床特点。方法修复角、巩膜伤口,玻璃体手术,内路摘出异物,剥除异物床区玻璃体后皮质,眼内光凝,封闭视网膜裂孔。临床随访评估手术疗效及并发症。结果38例(38眼)均摘出异物。术中共查见55个视网膜裂孔,裂孔成因分别为:①异物直接刺伤,异物弹跳二次撞击刺伤;②贯通伤发生裂孔;③不合理选择外路反复磁吸异物且失败。结论眼后段眼内异物及其创伤性视网膜裂孔应选择玻璃体联合手术,尽量去除诱发外伤性增殖性玻璃体视网膜病变的危险因素。

Objective Evaluation of the clinical characters and vitrectomy tor posterior segrnent intraocular foreign bodies associatecl traumic retinal tears. Methods Vitrectomy assisted with intraocular foreign bodies extraction, removing posterior vitreous cortex, endo- laser photocoagulation andclosing traumic retinal tears. The mean following - up period in these patients was 10 months, and their effects and complications were analysed. Results The posterior segment foreign bodies were removed in 38 cases. 55 traumic retinal tears were found during operation, which included dicrect punctured retinal tears, double punctured retinal tears, perforated traumic retinal tears, and multiple traumic retinal tears. The traumic retinal tears were closed and retinal detachment reattached. Conclusions We chose vitrectomy for treating posterior segment intraocular foreign bodies associated traumic retinal tears. It is important that the predictable or prognnostic dangenous factors what are traumic lens cortex, vitreous haemorrhage, pathologic vitreous, covered tissues of forgien bodies, posterior vitreous cortex are sectioned and removed. Also traumic retinal tears are closed and retina reattached.
