粒细胞集落刺激因子动员正常供者外周血造血干细胞研究

摘要 目的:探讨粒细胞集落刺激因子(G-CSF)动员正常供者外周血造血干细胞的效果、影响因素及不良反应。方法:对20例造血干细胞正常供者采用中位剂量G-CSF10.9μg/(kg·d)皮下注射3~5天后,使用COBESpectra血细胞分离机采集外周血干细胞。采用流式细胞仪检测采集物中CD34+细胞数。结果:第一次采集的单个核细胞(MNC,个)及CD34+细胞量(个)分别为(1.12~13.06,中位数3.10)×108/kg供者体重及(1.14~10.42,中位数3.8)×106/kg供者体重。MNC及CD34+细胞采集总量分别为(2.21~13.60,中位数5.80)×108/kg受者体重及(2.30~14.00,中位数6.40)×106/kg受者体重,均达到移植要求。男性CD34+细胞总量显著高于女性。不良反应较为轻微。供者年龄、体重、G-CSF剂量、采集前白细胞数等与采集所得MNC及CD34+细胞量无明显相关性。结论:G-CSF作为正常供者动员剂安全有效。 Objective: To study the efficacy, safety of G-CSF-based mobilization in normal healthy donors for allogeneic peripheral blood stem cell transplantation. Methods: G-CSF was used to mobilize peripheral blood hematopoietic stem cells (PBSCs) in 20 healthy donors and PBSCs were collected hy COBE Spectra cell separator. CD34^+ cells were calculated by flow cytometry. Results: The number of MNCs and CD34^+ cells of the first collection were (1.12-13.06,median:3.1) × 10^8/kg and (1.14-10.42,median 3.8) × 10^6/kg respectively per kilogram donor weight.The total number of MNC and CD34^+ cells were (2.21-13.60,median: 5.80) × 10^8/kg and (2.30-14.00,median 6.40) × 10^6/kg respectively per kilogram patient weight. Both the total numbers of MNCs and CD34^+ cells were enough for transplantation.Significantly more total number of CD34^+ cells were mobilized in men than in women. The side effects were mild. There was no correlation between donor age, body weight, the dose of G-CSF or leukocyte counts of peripheral blood and the number of MNCs or CD34^+ cells of the first collection. Conclusion: Mobilization with G-CSF in healthy donors is safe and effective.
