摘要
目的探讨慢性阻塞性肺疾病(COPD)患者夜间低氧血症的发生情况及血小板活化与肺动脉高压的相关性。方法55例稳定期COPD患者分为非呼吸衰竭组(A组,40例)及呼吸衰竭组(B组,15例);对照组(C组,15例)为急性肺炎治愈患者。进行睡眠时监测,用彩色多普勒估测肺动脉压,流式细胞仪测定血小板活化分子CD61、CD42b和CD49b。结果COPD患者夜间睡眠时出现低氧或低氧血症加重,伴夜间低氧血症患者的血小板活化增强。结论推测低氧所致血小板活化增强在肺动脉高压的形成过程中起重要作用。
Objective To determine the incidence of nocturnal hypoxemia in patients with COPD and the expression of actived platelets in these patients and to find potential relationship between actived platelets and pulmonary hypertension. Methods Fifty-five stable COPD patients were monitored with OPO(Overnight pules oximetry, OPO) for 7 ~ 8 h at night with air breathing. Pulmonary artery pressure and actived platelets were also measured (CD61, CIM2b, CIM9b). Results The nocturnal oxygen level obviously decreased in COPD patients and actived platelets obviously increased in COPD patients with nocturnal hypoxemia whether daytime PaO2 〉 9.0 kPa. Conclusion It may play an important role in the development of pulmonary hypertension.
