经腹经阴道行中下段直肠癌根治术

Radical resection of middle and lower rectal carcinoma by abdomen and vagina approach

作者应用经腹部游离乙状结肠和直肠,经阴道后壁横切口根治切除女性中下段直肠癌32例。瘤段肠管经阴道切口拖出直视下完成切除和吻合。随访6～70个月,无瘤牛存29例,占90.6%,局部复发2例,占6.25%。本术式的优点为:(1)设计合理、经路直接、创伤小。(2)显露清楚、切除彻底。(3)体外直视下超低位吻合、操作方便。(4)肛直肠环和肛管括约肌完整,排便功能恢复好。(5)减少盆腔污染。

Radical resection was performed in 32 female Cas- es of middle and lower rectal carcinoma through ab- domen and vagina.The rectal segment was pulled out through the vagnia incision,resected and anastomosed under direct vision.Patients were followed up for 6-70 months,29 patients(90.6%)survived without recur- rence,and 2 had local recurrence (6.25%).The advan- tages of the procedure include minimal trauma,clear exploration,very low anastomosis under direct vision, less contamination of pelvic cavity,and satisfactory postoperative defecating function.
