扪不到包块的乳癌16例诊治分析

The diagnosis and treatment of sixteen cases nonpalpable breast cancer.

分析16例扪不到包块的乳癌的诊断和治疗。结果显示在乳腺囊性增生病、乳腺导管内乳头状瘤早期癌变或导管内癌,乳腺派杰病和早期浸润性导管癌等均可在扪不到包块期发现。乳腺局限性增厚、病理性乳头溢液、乳头糜烂、新近发生的乳头轻度回缩、乳房皮肤轻度凹陷或水肿、一侧乳癌术后的对侧乳腺有可疑病变者或更年期后又出现乳房症状者是其较早期出现的症状,需进一步进行相应的检查及精确的定位活检。随访结果提示单纯乳房切除加腋窝淋巴结清扫术可作为治疗的基本术式,并根据肿瘤浸润、转移及雌激素受体等情况决定术后放疗、化疗和内分泌治疗。

The diagnosis and treatment of sixteen cases nonpalpable breast carcinoma were analysed. The results indicated that the early breast cancer at nonpalpable phase can be found in the diseases of breast cystic hyperplastic disease and intraductal papilloma with early carcinomatous transformation, intraductal carcinoma, Paget's disease and infiltrating ductal carcinoma. The most common symptoms observed in this groups were focal proliferation of mammary gland, pathological nipple discharge, nipple erosion, nipple retracted slightly, localized depression or edema of mammary skin, a suspicous focus in the contralateral breast found in postoperation for breast cancer and the breast symptoms occured in postmenopause, which should be followed by the further examination and the accurate biopsy. The results of 2～10 years of following up in this group suggested that nonpalpable breast carcinoma may be appropriately treated by mastectomy and axillary resection. The postoperation treatment depend on the conditions of the infiltration,metastasis and ER content of the tumors.
