双水平气道正压通气呼吸机治疗慢性阻塞性肺疾病合并呼吸衰竭

Clinical study of BiPAP ventilator used for chronic obstructive pulmonary disease combined with respiratory failure

目的评价双水平气道正压通气(BiPAP)呼吸机治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭的临床价值。方法比较76例COPD合并呼吸衰竭患者随机经常规治疗(对照组30例)和加用BiPAP呼吸机治疗(治疗组46例)的临床疗效、血气分析的变化及对心率、呼吸频率的影响。结果治疗组符合临床好转标准42例,对照组19例,两组比较差异有显著意义(P<0.005);两组治疗后4、24hPaO2、PaCO2、呼吸频率及心率的变化比较差异有显著意义(P<0.001),并且BiPAP呼吸机组鼻面罩患者依从性较好。结论正确应用BiPAP呼吸机治疗COPD合并呼吸衰竭能更快地改善患者的通气功能、纠正二氧化碳潴留和低氧血症。

Objective To evaluate the clinical effect of BiPAP ventilator used for chronic obstructive pulmonary disease （COPD） combined with respiratory failure patients. Methods Seventy-six COPD combined with respirtory failure patients were randomly divided into two groups：conventional treatment group（control n = 30） and BiPAP ventilator treatment group（treat n = 46）. The accord ratio of clinical improve standard, alter of blood gas analysis and effect on HR,PR were compared. Results The accord ratio of clinical improve standard was forty-two and nineteen respective in treat group and control group,two groups showed significant difference（P 〈 0. 005） ;The PaO2, PaCO2, HR, PR had notable difference after treatment in 4,24 hours（P 〈 0. 001 ） between two groups and the treat group had better compliance than the control group. Conclusion The ventilation function, carbon dioxide retention, hypoxemia will be rapidly improved when BIPAP ventilator is used correctly to treat COPD combined with respiratory failure.
