The Study of Blood Group Antibody of Newborn Hemolytic Disease

目的探讨ABO-HDN血型不合孕妇血清IgG抗A(B)效价与ABO-HDN的关系。方法对1 632例O型孕妇血清IgG抗A(B)效价检测及抗体分型分析。结果1 632例孕妇血清中IgG抗A(B)≥1/64者249例,占15.3%;ABO-HDN发生率随IgG抗A(B)效价升高而上升;HDN与妊娠史的频率有关,随着年龄上升,孕妇个体IgG抗A(B)抗体效价也随之上升。结论夫妇间ABO血型不合者应及时检查产前血型抗体水平;预防不良妊娠的发生;IgG抗A(B)效价的高低与HDN的发病率成正比;孕妇血清IgG抗A(B)效价≥1/64需定期检测抗体效价水平及时用药降低抗体效价,预防新生儿溶血病的发生。

Objective To explore relationship between the level of serum anti-A（B） IgG tier of pregnant woman with ABO blood group incompatibility and hemolytic disease of newborn （ABO-HDN）. Methods The level of blood group antibody anti-A（B） IgG tier and antibody subgroup were examined in 1 632 pregnant women with O blood group. Results 249 cases of 1 632 pregnant women showed the level of the serum antibody anti-A（B） IgG tier≥1/64,accounted for 15.3%. The incidence of ABO-HDN increased along with the higher of IgG anti-A（B） tier ;HDN gets along with the frequency of gestations. With the raise of age,anti-A（B） IgG tier of pregnant women was along with rising,too. Conclusion The altenatal blood group antibody level assay of ABGU in the gestational couples was in time. Defend happen of the bad gestation;anti- A（B） IgG tier≥1/64 in pregnant women should be tread with medicine. The level of blood group immunity antibody of IgG and the incidence of HDN was in proportion. The HDN can be predicted.
