牙髓应急治疗效果的对比研究

A Comparative Study of the Effect of Endodontic Emergency Treatments

对３１５例急性牙髓炎和２４０例急性牙髓炎合并根尖周炎采用开髓引流术和牙髓摘除术或牙髓切断术加失活法的封髓疗法，结果封髓疗法组疼痛缓解率明显高于开髓引流术组（Ｐ＜０．０１）；术后疼痛加重和急性根尖周炎发生率明显低于开髓引流术组（Ｐ＜０．０１），完成牙髓治疗的总次数明显低于开髓引流术组（Ｐ＜０．０１）。结果表明急性牙髓炎或急性牙髓炎合并根尖周炎时，采用开髓引流术的应急处理可致牙髓治疗复杂化，止痛不彻底，术后易出现急性根尖周炎，为了达到理想的疗效，急性牙髓炎牙髓应急治疗应去除炎症的牙髓组织和密封开髓孔。

The effect of two different kinds of endodontic emergency treatments performed on 315 patients with acute pulpitis and on 240 patients of acute pulpitis with apical periodontitis was compared. The results showed that when teeth were kept closed, the percentage of relief for pain was significantly higher than that of the teeth left open (P<0.01); In teeth kept closed at emergency treatment, the incidence of exacerbation of pain and acute periapical periodontitis was significantly lower than that of the teeth left open during the course of treatment (P<0.01). The total number of appointments needed to complete treatment after teeth had been kept closed at emergency appointment was significantly less than that of the teeth left open (P<0.01). The results indicated that teeth with vital pulps had been left open at emergency treatment, the later treatment would have been greatly complicated. Therefore in the condition of acute pulpitis and acute pulpitis with apical periodontitis, all the inflamed pulp tissues must be removed and the access cavity sealed to gain best results.
