摘要
目的:测定8~15岁单纯性肥胖与正常女生血清瘦素、胰岛素和雌二醇水平,分析其与体质量指数的关系。方法:①于2003-03/04在河南省荥阳市选择4所小学和2所中学,对年龄8~15岁的全部女生进行健康检查,筛选出单纯性肥胖女生60人作为肥胖组(监护人均对本调查均知情同意),按1∶1配对原则选择同年龄、同性别体质量正常女生60人作为对照组。将两组按年龄(以采样时间为界,计算实足年龄)划分为8~9岁,10~11岁,12~13岁和14~15岁4个年龄段,每个年龄段15人。②计算体质量指数[体质量(kg)/身高2(m2)]。采用放射免疫方法测定纳入对象血清瘦素、胰岛素和雌二醇水平。③计量资料组间比较确定方差齐性,采用方差分析;各种激素与年龄、体质量指数进行直线回归相关性分析和多元Logistic逐步回归。结果:单纯肥胖和体质量正常女生各60人均进入结果分析。①各年龄段肥胖组女生血清瘦素、胰岛素水平明显高于对照组女生(P<0.05);两组各年龄段女生血清雌二醇水平相近,雌二醇水平随着年龄的增长而升高。②瘦素与体质量指数均呈显著正相关(r=0.65~0.87,P<0.05),与胰岛素也呈显著正相关(r=0.57~0.93,P<0.05)。胰岛素与体质量指数仅在10~11岁和12~13岁年龄段呈显著正相关(r=0.70,0.63,P<0.05。雌二醇与瘦素和胰岛素及体质量指数均无明显相关性。在分别以瘦素和胰岛素为因变量,体质量指数、年龄、雌二醇为自变量的逐步多元回归中,体质量指数仍是影响瘦素、胰岛素水平最为显著的因素(r=0.48,0.43,P<0.01。结论:①8~15岁单纯性肥胖女生血清瘦素、胰岛素水平偏高。②血清瘦素、胰岛素水平与体质量有关。
AIM: To measure the levels of serum leptin, insulin and estrogen in girls with simple obesity and normal ones at the age of 8-15 years, and analyze the relation with body mass index(BMl). METHODS: ①A health examination was carried out in all the girl students of 8-15 years from four primary schools and two middle schools in Xingyang, Henan Province from March to April 2003. Among them, 60 students withsimple obesity were enrolled as obesity group (Informed consent was obtained from their guardians), and another 60 health girls were selected as controls in terms of age, sex and body mass at a radio of 1:1. All the subjects in two groups were assigned into four age group: 8-9 years old, 10-11 years old, 12-13 years old, 14-15 years old, with 15 subjects in each group.②BMI was calculated in all the subjects, and levels of leptin, insulin and estrogen were measured by using radio-immunity method. ③Analysis of variance was adopted for measurement data between groups, and the correlation of different estrogen to age and BMI was determined by using linear regression correlation analysis and multiple Logistic stepwise regression analysis. RESULTS: All the subjects were involved in the result analysis.①Levels of serum leptin and insulin of the obesity girls at different age were higher than those of normal health girls (P〈 0.05); however, there was no difference in serum estrogen level between the two groups, and the level of serum estrogen increased along with age. ②Leptin level had a remarkable positive correlation with BMI and insulin level (r=0.65-0.87, P〈0.05; r=0.57-0.93, P〈 0.05). However, a positive correlation between insulin level and BMI was only found in the subjects at the age of 10-11 and 12-13 years (r=0.70, 0.63, P〈 0.05). There was no correlation between estrogen level and other indexes. If leptin and insulin served as dependent variables and BMI, age and estrogen as independent variables, BMI was still one of the most significant influencing factors for levels of leptin and insulin(r=0.48, 0.43, P〈0.01). CONCLUSION: Levels of serum leptin and insulin are correspondently high-er in girls of 8-15 years, which are correlated with BMI.
