强直性脊柱炎与HLA－B27亚型

ＨＬＡ－Ｂ２７是一组紧密相关抗原的总称，根据它们之间个别氨基酸的差异，目前ＨＬＡ－Ｂ２７抗原可至少分为６种亚型（Ｂ２７０１－Ｂ２７０６）。采用聚合酶链式反应和寡核苷酸探针斑点杂交方法，在中国汉族人群中，对１２００例正常人及７６例强直性脊柱炎患者ＨＬＡ－Ｂ２７基因进行了分析。结果显示，该基因与强直性脊柱炎易感性显著相关，在汉族人群中至少存在四种亚型，其中Ｂ２７０４分布频率最高，分别占ＨＬＡ－Ｂ２７阳性的强直性脊柱炎患者和正常人群的５４．８％和５０．８５％。在正常人和强直性脊柱炎（ＡＳ）患者中，ＨＬＡ－Ｂ２７主要亚型的分布频率无显著差异。我们认为，不同人种ＨＬＡ－Ｂ２７亚型的分布频率不同；各亚型共同序列特征。

HLA-B27representsafamilyofcloselyrelatedantigens.Morethansixalelesdiferinginlimitednumberofnucleotidesubstitutionhadbeendescribed(B2701-B2706).WesubtypedHLA-B27in76patientswithankylosingspondylitis(AS)and1200ChinesehealthypersonsofHannationality.PolymerasechainreactionwasdesignedtoamplifyspecificregionsforclassI(exon2and3).Amplifiedsequencesweretestedwitheightsequence-specificoligonucleotidestodistinguishalHLA-B27sub-types.Theresultsshowed:theHLA-B27wascloselyrelatedwithASandatleastfoursubtypesexistedinChinesepatients.B2704wasthemostcommonsubtypeoccurringin54.8%and50.85%respectivelyinHLA-B27positiveASpatientsandhealthycontrols.ThemainHLA-B27subtypesintheHLA-B27positivepatientsdistributedinsimilarproportiontothatofHLA-B27positivehealthycontrols(P>0.1).WebelievethattheaboveresultsimplythattheprevalencesofthevarioussubtypesofHLA-B27varyindiferentraces.TheHLA-B27specificresiduesthataresharedbythesubtypesmightbeinvolvedinthepathogenesisofAS.
