老年白血病的临床特点（附38例临床分析）

An analysis of 35 cases of leukemia in elderly

本文分析了１９８５～１９９４年间收治的老年白血病３８例。其中急性白血病２７例，慢性白血病１１例，占同期白血病患者的０．０９。其特点：０．７６患者起病有头晕、乏力症状，０．６７有发热症状，骨髓增生活跃至极度活跃者占０．９５．并发症以感染（占０．７１），骨髓抑制（占０．４５）及出血（占０．２４）多见。少数病人因诊断前病程长、症状不典型、末梢血中少见或不见白血病细胞等易造成误诊或漏诊。老年白血病患者对化疗反应差，缓解率低，生存期短，死亡率高。显示老年白血病具有一定的临床特点。

At the very beginning,0.76 pa-tients had the experiences of dizzy and weakness,0. 67patients experienced fever,0.95 patients showed a mod-erte or extreme myelosis. Complication infection occurredin 0. 71 patients, Secondarily deprerrsed myeloid metapla-sia in 0. 45 patients and hemorrhase in 0.24 patients. fewpatients were misdiagnosed or escaped from diognsis dueto lacking typical Symptoms, having few or no Leukemiacell in peripheral blood or possessing an entirely asymp-tomatic or insidious onset before consulting the doctors.The elderly leukemia patients showed a lower sensitivityto chemical drugs、depressed remission rate、shorter sura-vival duration and higher mortality、indicating hat theclincal course of the elderly leukemia patients is quite dif-ferent from that experienced by younger patients.
