输液对呼吸道烧伤犬血浆量和电解质等的影响

Effects of Intravenous Fluid on Plasma Voiume and Electrolyte Contents after Respiratory Tract Burns in Experimental Dogs

摘要 本实验针对补液治疗呼吸道烧伤的各公式进行研究,发现输液量仍显不足,并对补液的质的问题作了简要讨论。 The experiments deal chiefly with the study of effectiveness of different formulae of fluid therapy in resuscitating dogs engendered with third degree 30% TBSA flame burn and severe respiratory tract steam burn. The results reveal that in all 4 groups of dogs, at the end of 24 hours postburn, as compared with their preburn values, there are decrease of plasma volume and electrolyte levels (as corrected with Ht), and increase of Ht, urine osmolarity and urine/serum osmolarity ratio, iadicating the presence of hemoconcentration. These implicate also that the amount of infused fluid calculated from the conventional formula, even from the Parkland's one, would not he sufficient to replenish the fluid loss, either in fluid amount or electrolyteS, in burns associated with inhalation injury. This is because that the fundamental principles of estimating the total amount of fluid replacement given in any formula are chiefly based on the extent of burn in relationship with TBSA which does not include the loss from the respiratory tract. Such estimation is by itself a below standard one and restriction of infusion seems not necessary in such cases if calculated amount of fluid is infused according to a conventional formula, It is noteworthy to find that the experimental results do not verify the common concept that actdition of colloids in the infusion regimen would make the dog's final condition better.
