东北汉族人群resistin基因多态性与2型糖尿病慢性并发症的关系

Relationship of resistin gene polymorphism with the chronic complications of type 2 diabetes mellitus in Han population of northeast China

摘要 目的:分析中国东北汉族人群resistin基因单核苷酸多态性与2型糖尿病慢性并发症之间的关系。方法:①选择2001-01/2002-12在哈尔滨医科大学附属第二医院内分泌科住院的2型糖尿病患者500例。均了解实验目的,并自愿接受。②采用葡萄糖氧化酶法测定血糖,采用全自动生化分析仪测定血浆总胆固醇、三酰甘油、高密度脂蛋白胆固醇及低密度脂蛋白胆固醇,采用化学发光仪测定血浆胰岛素及C肽,采用全自动糖化血红蛋白分析仪测定糖化血红蛋白。③从纳入的500例患者中随机选取30例2型糖尿病患者,采其外周静脉血4mL,提取外周血DNA;并用聚合酶链反应法对resistin基因5’端行选择性扩增;对聚合酶链反应产物进行纯化后进行测序。④采用SAS(Cary,NC)软件分析resistin基因单核苷酸多态性与哪些因素相关及这些因素与2型糖尿病慢性并发症的关系;两组均数间比较采用t检验。结果:2型糖尿病患者500例均进入结果分析。①发现resistin基因5'端调节区有5个多态位点,分别为:g.-638G>A,g.-537A>C,g.-420C>G,g.-358G>A,g.-238G>A。②具有g.-238G>A多态位点的2型糖尿病患者舒张压明显高于无该多态位点者(87.50±3.91,79.82±1.71,P<0.05)。③2型糖尿病患者心血管并发症、脑血管并发症、眼部并发症及神经病变均与舒张压呈显著正相关(检验统计计量为3.9483~18.2319,P<0.05~0.01)。结论:中国东北汉族人群中有g.-238G>A多态性的2型糖尿病患者更易发生心血管并发症、脑血管并发症、眼部并发症及神经病变。 AIM: To analyze the relationship of resistin gene single nucleotide polymorphisms (SNPs) with the chronic complications of type 2 diabetes mellitus in northeast China Han population. METHODS: ① 500 type 2 diabetics patients, who were treated in the Department of Endocrinology, Second Affiliated Hospital, Harbin Medical University, were selected. They all knew the aim of the experiment and accepted it voluntarily. ② The blood sugar was detected by the glucose oxidase method. The plasm total cholesterol, triacylglycerol, high-density lipoprotein (HDL) and low-density lipoprotein (LDL) were detected with the automatic biochemistry analyzer. The plasm insulin and C peptide were detected with the chemiluminescence apparatus. The glycated ferrohemoglobin was analyzed with the automatic glycated ferrohemoglobin analyzer.③ Among the 500 included patients, 30 patients with type 2 diabetes were collected randomly. The peripheral venous blood of 4 mL was gained and the DNA was separated from peripheral blood. The resistin gene 5-tip was extended selectively with polymerase chain reaction (PCR). The sequence of PCR products was measured after purification.④ The factors related to resistin gene SNPs and the relation of these factors with chronic complications of type 2 diabetes were analyzed with the SAS(Cary, NC) software. The means of the two groups were compared with the t-test. RESULTS: 500 patients with type 2 diabetes were all involved in the result analysis. ① There were 5 polymorphism sites in the regulatory domain of resistin gene 5-tip: g.-638 G 〉 A, g.-537 A 〉 C, g.-420 C 〉 G, g. -358 G 〉 A, g.-238 G 〉 A, respectively. ② The diastolic pressure in the type 2 diabetes patients with g.-238 G 〉 A polymorphism site was higher significantly than that in patients without the polymorphism site (87.50 ±3.91, 79.82 ±1.71, P 〈 0.05). ③ The complication of cardiovascular disease, cerebrovascular disease and eyes, and the neural lesion had significantly positive correlation with the diastolic pressure (the result of check statistics: 3.948 3-18.231 9, P 〈 0.05-0.01 ), CONCLUSION: The type 2 diabetics patients with g.-238 G 〉 A polymorphism in the northeast China Han population are easy to get the complication of cardiovascular disease, cerebrovascular disease and eyes, and neural lesion.
