腰背部损伤所致的腹痛

Abdominal Pain of Lumbodorsal Injury

本文报告由腰背部损伤所致腹痛46例。全部病例均为急性损伤,以腰肌损伤为最多,压缩性骨折和挫伤次之。损伤部位以胸_(12)～腰_3为主,腹痛部位以下腹和脐周多见。体检时腹部轻度压痛36例,91.7%的患者无腹肌紧张。在作出诊断时,必须慎重排除腹内的实质性病变。治疗应以腰背部原发损伤为主,椎旁肌封闭能收到暂时的效果。

Forty-six cases of abdominal pain of lumbodorsal injury from Oct 1978 to March 1986 in our hospital were reported. There were 38 males and 8 females. Age ranged between 20 and 40 in 89.8%, The clinical features were： （1） All patients had a history of lumbo dorsal injury associated with mild lumbagor （2） The pain was mild and constant, mainly localized in hypogastric and periumbitical regions; （3） A soft abdomen with mild tenderness; （4） Local block of the tender point over the loin may give relief to the abdominal pain The condition is often misdiagnosed as acute appendicitis, traumatic injury of abdomen and acute cholecystitis oct. Treatment consists of removing primary lesion and local black in the loin An investigation on the result and mechanism of abdominal pain of lumhodorsal injury is discussed.
