罗哌卡因颈丛神经阻滞中惊厥一例 被引量：5

摘要 患者,女,39岁,体重47kg,身高159 cm,ASAⅠ级.拟在颈丛神经阻滞下行甲状腺瘤摘除术.既往无癫痫等病史,术前心、40、100μmol/L依托咪酯浓度时其抑制率分别为24%和33%.肺、肝、肾功能和血常规未见异常.麻醉前30 min肌肉注射阿托品0.5mg、苯巴比妥钠0.1 g.入手术室后建立静脉通路,监测无创血压(BP)、心电图(ECG)和脉搏血氧饱和度(SpO2),BP 105/75mmHg(1 kPa=7.5mmHg),心率(HR)96次/min,SPO299%.患者去枕平卧位,头转向右侧,在胸锁乳突肌后缘与颈外静脉交叉点附近定位第4颈椎横突,并在其上方约1.5 cm处定位第3颈椎横突.
